Treat the questionnaire as the start of a question
The posted 2015-era questionnaire distinguishes receipt from program acceptance. The indexed Apply For resource names the Home Improvement Program, but its full page was blocked during this check. Neither observation establishes current intake or an award.
Start the conversation by identifying the property and unit count, then ask which current form and program route applies. Do not send private financial documents through this directory. A form found online should not become the basis for assuming that Housing has reviewed the household or the proposed work.
Keep the repair scope separate from an award assumption
Prepare a short account of the conditions you want assessed, with the affected rooms or exterior features identified. Keep any earlier assessment or estimate available as background, clearly separated from work already authorized and work merely being considered.
Ask Housing what may be commissioned while an inquiry is being reviewed and what decisions must wait for its direction. If you also seek a private proposal, describe that proposal as a separate scope discussion. A provider’s price or willingness to undertake repairs does not determine the city’s assistance decision.
Does South Gate’s receipt of an initial questionnaire mean the repair is approved?
No; receipt is not acceptance. Ask Housing about the present process and the actual application before making any work dependent on an assumed award.
Should I use the posted older form without checking with Housing?
Check first: the posted document is older. Ask Housing which current form to use and where supporting information belongs; no current submission or assistance has been confirmed here.

For a rented home, confirm who can authorize alterations. In a shared building, ask about access and work rules. A citywide construction period does not establish the age, condition or safety requirements of an individual home; check the property records and the specific project.
