Locate the walking-surface segment precisely
The official Streets and Sidewalks excerpt names Report an Issue for a sidewalk tripping hazard. Direct access returned a block during this check, so the full reporting instructions were not available. Use the current city resource to clarify how to report the observed location.
Make the report about the condition someone encounters on the walking route: the particular surface change and where it sits relative to an identifiable address or crossing. Keep a dated view that makes the location recognizable. Describe the observation rather than assigning its cause, repair responsibility or accessibility status.
Separate the report reference from the work order
Retain the report reference so a later update can be connected to the same hazard observation. The report is a public-works inquiry; it is not an acceptance into a repair program, a scheduled crew visit or permission for a private provider to alter the sidewalk.
If the household also wants an entry step repaired, write that as its own work request. Identify the owner-authorized feature and the assessment needed there. Keep the city reporting question visible beside that private task instead of allowing one general access-repair quote to stand in for both decisions.
Should I describe the street sidewalk and my entry steps as one repair location?
Label the two segments separately. The street-side observation belongs in the relevant city reporting inquiry, while the entry-step proposal needs its own location, assessment and authorization.
Does a Champaign sidewalk report establish that a private provider may repair it?
No such authorization has been established. A report identifies a concern for the city; ask about the actual next step and keep any privately authorized work separately defined.

For a rented home, confirm who can authorize alterations. In a shared building, ask about access and work rules. A citywide construction period does not establish the age, condition or safety requirements of an individual home; check the property records and the specific project.
